Seasoned Hearts Foundation Membership FAQ

How does a student become a member?
All students must complete a Student Membership Application (SMA) prior to participating in any Seasoned Hearts Foundation programs or events. This application must be signed by both the student and a parent/guardian.

What is the age range of teens that you serve?
Seasoned Hearts Foundation serves students currently in 7th–12th grade, who are at least 12 years old and under 19.
Students entering 7th grade in the fall (and who are at least 12) may begin attending during the summer after completing 6th grade. -
Where can I get a Student Membership Application (SMA)?
The application is available online [HERE] or can be picked up in-person at the Seasoned Hearts Foundation Center.

Why do I need to fill out the SMA?
The SMA ensures we maintain a safe and welcoming environment for all students, staff, and volunteers. The information provided—such as medical conditions, allergies, and emergency contacts—helps us respond quickly in case of an emergency.
Note: Your personal information is never sold or shared. Anonymous demographic data may be used in grant applications to help fund our programs. -
Does the SMA need to be completed every year?
No. The SMA only needs to be completed once and is valid through the student's high school years.
If information changes, please contact us via email or phone to update your student’s record. -
My cousin from out of town is visiting. Can they come with me?
Yes! Visiting students are welcome if they bring a completed SMA. If their parent is unavailable to sign, a local guardian may sign instead.

Can I update my student’s information after the SMA is submitted?
Yes. You can update information anytime by emailing us at SeasonedHeartsFoundation@yahoo.com or calling (312) 436-1752.

Who is eligible to attend the Seasoned Hearts Foundation Center?
Students in 7th–12th grade who have submitted a completed SMA and meet our eligibility guidelines are welcome.
Students may begin attending in the summer before 7th grade and continue through the summer after graduating high school or until their 19th birthday.

Are there residency restrictions?
No. While our center is located in Chicago Heights, we welcome students from all communities.

Will my student need money when attending programs or events?
Most of our programs are free or reduced cost, thanks to generous donors and sponsors. Some special activities may have a small fee, which we work to keep affordable.
Optional purchases include:- Snacks: $0.50 – $1.50
- Beverages: $1 – $2
- Seasoned Hearts Apparel: $5 – $25
- Snacks: $0.50 – $1.50

How does my student get to and from the Foundation Center?
Students typically arrive by:
- Parent/guardian drop-off
- PACE bus
- Walking or biking (weather permitting)
Note: For safety reasons, students are not permitted to come and go freely unless they drive themselves. Walking or biking after dark is discouraged unless prior arrangements are made with a parent/guardian and approved by staff.
We also offer a "Seasoned Hearts Bus" that picks students up from select local schools.
